The Swanson/OHSU Wellness Program respects your personal privacy and is committed to adhering to federal and state privacy laws and industry guidelines in order to protect you and your identity. The OHSU wellness staff will know if I have particular risks or conditions, however, this same information will NOT be shared with my employer, my supervisors, or anyone else at Swanson. All personal health data will be kept separate, secure and in a confidential database. The information that Swanson Human Resource will receive will be delivered in aggregate (group summary) format and will exclude any employee or spouse names or other identifying information.

*Only exception to absolute confidentiality is if physical threat made to self or others.
